[A] Budget: Looking for proposal
[B] Scope of Service:
Vendor needs to provide pharmacy benefit management services to provide pharmacy services to eligible veterans seen by authorized prescribers assigned to following primary care clinics and community based outpatient clinics.
- This statement of work is limited to the provision of outpatient prescriptions for urgently needed authorized formulary medications to patients seen at after normal business hours, weekends and holidays.
- Prescriptions filled must be called into Pharmacy or electronically printed on contract pharmacy prescription voucher.
- Prescriptions shall be filled and dispensed only for medications in formulary “drug list” authorized at the southern state healthcare system.
- All prescriptions shall be filled for quantities up to a maximum of ten days only.
- The prescribing provider may elect to write the prescription for less than ten days, and the pharmacy may only fill for prescribed number of days. This will provide a sufficient time for the VA to activate the monthly fill process where necessary.
- Prescriptions filled under this contract shall be generic drugs to the extent permitted by law. The least expensive drug product the pharmacy has in stock shall be dispensed. Generic drugs shall be dispensed if in the professional judgment of the pharmacist, the substitute product is available, safe, effective, and to the greatest extent possible, contains the same chemical ingredients of the same strength, quantity, and dosage of the brand name.

- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
