[B] Scope of Service:
(1) Vendor needs to provide occupational health services include: primary services, annual physical exams for police officers and firefighters cardiac, pulmonary, audiology consultations or evaluations and hepatitis inoculations and screening. Secondary services include: pre-placement medical exams, physical assessment testing, CDL exams, respiratory compliance exams, drug and alcohol testing, blood borne pathogen post exposure services, and miscellaneous occupational health services as identified during the course of the contract to the government authority located in Carson, NV.
(2) All questions must be submitted no later than January 17, 2020.
